What Medical Condition Could Cause Blood In The Urine?

I am seeing blood in my urine and at times what appears to be clots a little smaller than the size of a dime. This has happened about once a month, so I thought it was due to some sort of spotting, vaginally; however, it appears to be in my urine. I do not have any bladder spasm symptoms, like I would with a UTI. Any idea what might be going on?
