Hi,

From what you have described, this doctor has put you on hormonal tablets combination:
- Primolut N, an anti-inflammatory pain medicine
- Ponstan Fort, and tranexamic acid to control bleeding

This is a very commonly used combination in women who present with sudden heavy bleeding and no structural problem in the uterus. So, the idea behind the treatment is medically correct. But the most important point here is how your body is reacting. Severe stomach discomfort and strong back pain after the start of these medicines are not something you should neglect. Hormonal tablets like Primolut N can cause bloating, nausea, mood swings, breast heaviness, and sometimes back pain.

Ponstan Fort also generally causes stomach irritation. When both are taken together for many days, side effects become more evident. So, your symptoms are not unusual, but they are strong enough that your treatment plan might need an adjustment. Is this the best course? Well, the overall plan is reasonable, but if the side effects are affecting your daily life, then it's not the best plan for you. Treatment has to be tailored for the patient and not just the textbook.

Regarding switching to Progyluton, Progyluton is another hormonal cycle-regulating pill. But it is not automatically "better" or "safer" than Primolut N. Whether it will suit you better depends on your hormone levels, ultrasound findings, and your general health. Many women have a very good tolerance for it, while others do not. So, it is not something I would recommend that you switch to on your own.

A gynecologist can decide after seeing your symptoms and probably ordering a few hormone tests.
At your age, 45 years, perimenopause is very often the cause of irregular and heavy bleeding; hence, sometimes treatment needs a little trial and adjustment. But before changing anything, I would strongly advise a follow-up visit because:
• Heavy bleeding that persists at this age should be re-evaluated
• Your side effects from the current regimen need attention.
• Sometimes, doctors adjust the dose or duration or switch over to another hormonal option according to the tolerance.

For the time being, do not suddenly stop all medicines without prior consultation with your doctor-especially the hormonal tablets-because that may trigger more bleeding. But definitely contact your gynecologist soon and tell her exactly what side effects you are having. She might modify the regimen, switch to a gentler hormonal option, or shorten the duration. If the bleeding is very heavy again, with rapid soaking of pads, dizziness, and weakness, please don't wait - seek urgent care.

Take care. Hope I have answered your question. If you have any further query I will be happy to help. Wish you good health.
