Hi there,
Height is purely determined by genetics,so there is no 'ideal' height for a given age.
weight is dependent on your frame(to find out your frame use Dr Radha's method above)
Once you have found out your height & frame, find out your ideal weight with the chart given here http://www.iloveindia.com/fitness/weight-loss/healthy-body-weight.html

The weight and height are not uniform for all depends whether you are small frame, medium frame or large frame.The easy way to find your frame type is -put your thumb and your middle finger around the smallest part of your wrist, if your thumb and middle finger do not touch, you have a large frame. If they just touch, you have a medium frame. If they overlap, you have a small frame. Let me know the results.
