General Surgeon 's  Response
Hi,

The most possible issue with blood in urine, testicle and back pain is calculus or gravels in the urinary system as per my surgical experience.

In our clinic, if x-ray and ultrasound are non conclusive, we suggest CT scan with Intravenous Pyelography for confirmatory diagnosis.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel
