What To Do For Kid Having Problem To Pass Bowel Even Though Taking Osmolax?

MY 23 month old has always had bowel issues and has spent the last 18months on ozmolax, over the last few weeks i have had to increase this as he has had alot of trouble passing. for the last 2 days he has tried and tried to push, screaming each time and passes nothing, he does have an old pile and recently a new one, doctors just said increase meds. Is there anything I can do to help him pass something, or is it time to head to the hospital for assistance?

He should take more vegetable, water intake should increase. At bedtime he can take 10ml liquid paraffin.Should not take non-veg for sometime.For piles he should have sitz bath.Apply proctosedyl ointment in anal canal.With this he should improve.Bulk purgative like husk of Isabgol can be tried.
