Whats The Cause For Difficulty In Passing Motion?

I usually have to pass motions after my every meal and have to sit for a lot of time and have to give a lot of pressure for the motions to release.after passing the motions a still feel something is still left.Can you please advice me what should I do to get rid of this irregular bowls.....

- General condition related as obesity , hyper cholesterolemia , chronic
smoking induced , sedentary life style

* Need thorough history , physical examination , necessary tests of lab
Ultrasound report and scopy in selected cases .
