Your query indicates that you are suffering from
anal fistula and inability to swallow solid food and fluids.
The cause for the anal fistula has to be found out.
In developing countries,
tuberculosis is the most common cause of fistulas in the peri-anal region.
In western countries,
crohns disease is the commonest cause of peri-anal fistulas.
The tract needs to be excised and examined microscopically to look for the disease process in the eurrounding soft tissues.
Secondly, the inability to swallow ffod and fluids can be due to obstruction at the upper gastrointestinal tract.
An upper gastrointestinal tract
endoscopy needs to be done to find out the cause for the symptoms. A biopsy may also be required at the same time.
Enteral or parenteral nutritional need to be given to maintain the nutritional status of your brother.
You need to
consult a gastroenterologist and gastrosurgeon for excision of the fistula, upper gastrointestinal endoscopy and biopsy.
