Question: 32 year old male. I have a rash on the lower to underside of both butt cheeks. it is bumpy and scaly. It does itch. i have tried anti fungal cream to no avail.

I have read your query.
I understand your concern.
I have seen the attached photographs also.
From the pic, the most likely possibility is fungal infection of skin.
You have mentioned that you have used antifungal cream. I would like to know which antifungal cream have you used and for how long?
At times, only topicals do not help.
With topicals , oral antifungals are also advised like fluconazole / terbinafine / itraconazole.
Now a days , resistance has emerged to the most commonly used antifungals. So it takes relatively longer time to get cured.
You should go for KOH mount to look for fungal elements.
I will advise you to visit your doctor so that he can order test for you.
You can share my opinion with him.
