Brief Answer:
Dr XXXXXXX XXXX opinion is apt

Detailed Answer:
Hello XXXXXXX

I read through your query and reports.

You had gone to right doctor, Dr XXXXXXX XXXX. His prescription according to me is really apt. You should take Vitamin B12, D, Calcium supplements. Physiotherapy is going to be key for next few weeks.

The reason for your problem is bone loss. Low calcium, Low Vit D leads to low absorption of calcium. So you should take these supplements injection for 3 months and a maintainence dose for 9 months.

Please see a good physiotherapist referred by your doctor.

Gathered information that injections are very painful , under what circumstances, these are administrated? why can,t i take only tablets. Now with b12 vitamin tablet Matilda forte, i am not comfortable due to reactions.I am afraid if i take injection, in addition to pain i may have reactions. Is it required dosage for one continuous year? I have attached blood test reports, both found to be in order then why should I take such huge dosage? Kindly check reports and tablets proscribed, can U suggest milder B12 tablet to avoid reaction

Thanks

Brief Answer:
The doses are standard

Detailed Answer:
Hello, Welcome back to HCM.

I am not sure who told you about painful injetions. The Vitamin B12 dose is only 3 ml to 5ml per injection and it is given to muscle or Intravenous. If you are afraid of injections you can take tablets of Vitamin B12 up to 1500mcg per day once a day. However the injection has better availability to correct the dose if deficient while in the oral tablet the bioavailability reduces.

There would not be any reactions, MECOBAL PLUS OD 1000 to 1500mcg is good.

Regarding Vitamin D- you have hypovitaminosis D, the treatment that was already given to you was standard. It was 60000IU per week for 6 weeks. If you take 440 IU the vitamin D will up by 0.5ug, so to cover the deficiency and also give a maintainence the dose prescribed was good.

Let me know if you have other questions.

1) As per the test report of Homocysteine, plasma is 12.62 which is well within ref range provided in the report which is 5.08-15.39. Then why should I take supplement?

2) Also I checked in web site of Tab Matilda and dosage recommended by drug company is for 2-3 months. In my case it is recommended for one year?

3) My doubt is even when result is within the range and why such long dosage?

4) In case of Vitamin-D & Calcium , I gathered info that there can be kidney stone formation if it is excessive dosage. How do i know how much to take?
will there be any symptoms? Curing of one discease should not lead to another one. I am not sure excessive dosage of vitamin-D Or calcium will lead to stone formation. Pl advise

5) Does shoulder weakening will lead to neck pain as i am having neck pain also

6) Any recommendation to take specific food which will prevent future occurrence?

Thanks for answering my earlier queries

Brief Answer:
I answered your questions one by one

Detailed Answer:
1) You do not need Homocysteine supplements. Neither your doctor nor I suggest them. He ordered only serum levels of homocysteine

2) The doctor expects Vitamin B12 insufficiency in all the persons over 60 yrs and thus a maintenance dose for a year. Same is the case for VitD3 levels.

3) Where is the Vitamin B12 test report? You attached only Homocysteine and Vit D3 only

4) No problem. The excessive HypervitaminD and calcium are extremely rare in clinical practice on supplementation. If a person take 10000IU of Vit D for 5 months continuously then there is a rare chance. Your prescription is much less than this.

5) Yes

6) No specific dietary measures needed. Calcium, Iron, Fibre rich diet
