Suggest Treatment For Distended Stomach And Urinary Incontinence

Question: Hi my mother is 84yrs old and has a very swollen stomach extremely distended she is as big as a nine month pregnant woman she cant go to the toilet only a tiny bit of fluid comes out of her bowels every other day, she is wearing pads as she wets herself all the time she now has a rash and stinging from the pads and she will not go to see a doctor as she is scared and agrophobic I called an ambulance for her last week and they told her she had to go to the hospital but she would not go I do not know what to do. She saw her local doctor with my sister and she told him that she is going to the toilet normally now and that everythings going good now so he let her go home is that wrong of him, I think it is when I rang him to tell him about her condition he was the one who told me to ring an ambulance please help me. Sorry to know about your mothers present condition.

Based on your query, my opinion is as follows:

1. Do consult her local doctor once again, regarding the present scenario. The position might have improved from previously.
2. Distended abdomen could be due to fluid accumulation. Bladder or bowel obstruction, would not cause so much distension. Fluid accumulation in the abdomen, usually would not affect bowel and bladder habits.
3. As, she is going to toilet normally now, not to worry about her bowels. Need to increase fluids and fiber in diet to improve it.
4. Regarding distended abdomen, need to watch for jaundice - yellowishness of eyes, skin. If present, will require hospitalization. If not, possibly not fluid accumulation and it could be due to usual fat accumulation due to reduced activity.
5. If necessary, an ultrasound abdomen can be done - to look for any abnormalities or a serum protein and serum bilirubin levels estimation.
6. Its difficult to convince her due to agorophobia. Review by your local doctor and clinical signs, if necessary with tests can help in worry reduction or further intervention.
