To recapitulate: Male/33 - pain by butt crack - read the history well and seen the pictures you have provided.
This looks to be folliculitis or seborrhoic dermatitis with infection.
I would suggest you the following:
- Stop using cotizone, as it is not indicated.
- Wash the area with plain water, avoid using soaps as they contain chemicals.
- Make the area dry with cotton cloth or better with hair dryer.
- Apply Solution Povidone Iodine locally to keep the area sterile to avoid secondary infection.
- Consult your Doctor to get a prescription of an antibiotic and multivitamin.
- Avoid hot and humid areas/atmosphere.
- Use loose underwear and pants.
- Allow aeration wherever possible like when you are at home.
- Avoid scratching or harsh rubbing the area.
All these things will help for the cure of the condition.
