What Causes Chronic Anxiety, OCD And Panic Disorder?

Question: Hello, I am a 34 year old male with chronic anxiety and was recently diagnosed with OCD and panic disorder. I previously tried Buspar with really bad results and quit the medication after six weeks of use. During the course of treatment, I developed extreme worry, agitation, nervousness, wrestlesness, panic attacks, and suicidal ideation. I do not suffer from depression and have never been suicidal.

My doctor more recently prescribed Paxil CR 12.5 with Ativan to be taken in the morning and evening for the first week of treatment at .5 mg.

I am writing because I am terrified to take the Paxil for fear of repeated symptoms experienced on Buspar. Will the Ativan help to curb these symptoms if they happen?

I was on Paxil once, ten years ago with great results. My experience with Buspar has conditioned me to fear other courses of treatment.

I can understand your concern. You have been diagnosed with OCD and panic disorder. OCD and Panic both are anxiety disoders that need treatment with medicines in high doses especially of SSRIs. Buspirone is commonly prescribed for symptomatic control of anxiety. It is known to worsen anxiety symptoms in some individuals.

Paroxetine is a SSRI and 12.5 mg doses are not very high doses for drug. This may result in some anxiety and restlessness for first few days of start of the drug. But you have already tolerated Paroxetine previously so as per my opinion you would not suffer worsening of these symptoms. Mild to moderate restlessness, agitation, panic attacks would reduce with doses of Ativan or Lorazepam. Take the doses of Paroxetine in evening time before going to bed after meals. This would help in good sleep and you will feel better with in a week of therapy. As per my opinion as the doses are in CR or controlled release form so taking the drugs in two times a day will not result in much benefit. Discuss with your doctor and take the drug doses in evening time only as Paroxetine is known to cause sedation. Secondly start with 12.5 mg per day doses only for about a week and then as per prescription increase the doses to 25 mg per day. This would reduce any increase of symptoms. Take the drugs with proper prescription only.

Taking the Ativan doses about half hour before taking Paxil before going to bed would control any restlessness and anxiety. This will alleviate any anxiety and panic attacks caused by Paxil.
