Brief Answer:
probable anxiety attacks.

Detailed Answer:
Dear XXXXXXX
I have read your details.

Keeping all the details in mind, there are few possibilities for your health issues.

It can a part of panic or anxiety attacks.
The reason why I say so is because:
1. Your most investigations(especially cardiac) are normal. In light of normal tests, the most probable cause becomes anxiety.
2. You have workplace stress which can be a contributing factor.
3. Tobacco is a known precipitant of anxiety attacks.
4. Your episodes of breathlessness appear out of no where and settle down in few seconds to minutes. This is a classical feature of panic attack.

Other possibility is of sleep apnea because:
1. Obesity is known precipitant of sleep apnea.
2. Breathlessness disturbing sleep is common symptom of sleep apnea.
3. You feel lazy and sleepy in day probably because your sleep remains incomplete due to disturbance in night.

Regarding management of your problem, i have some suggestions:
1. A sleep-EEG will help to rule out sleep apnea.
2. Practice deep breathing regularly in routine. This will train your respiratory sytem to better adapt to episodes of breathlessness.
3. Other relaxation therapies like biofeedback and meditation can help.
4. Drugs like klonopin and paroxetine (prescription drugs) can help to overcome stress and anxiety.
5. Eat healthy food including green vegetables and fruits.
6. Try to reduce your weight.
7. Find ways to destress yourself, listening to music, reading, playing etc.

Consult a nearby psychiatrist for proper prescription and evaluation.

Hope this helps.

Feel free to ask further.

Thanks.
