What Causes Headache, Nasal Congestion, Sneezing And Ear Pain While Having Sinusitis?

Question: my sister is 22 yrs old , known case of Sinusitis, yesterday on a last visit to Out patient clinic after her symptoms was aggrivated frontal headache mostly when bending down while praying and also maxillary headache , Nasal congestion and sometimes sneezing , her condition complicated to otitis media (Rt mid Ear Pain) , the doctor just prescribe to her only Rhinocort Aqua 64microgram Nasal Spray BID
I need your opinions about the appropriate treatment plan for her condition that we can started ?
there is any need to take an antibiotics , anti allergic medications , other treatment?
I need your educational Advice tips to prevent the relapse and could be helpful beside treatments?

1. I recommend a plain CT PNS (Para Nasal Sinuses) with 1 mm coronal, axial and sagittal cuts. A plain CT PNS (Para Nasal Sinuses) will definitely help understand her problem. She also has symptoms of allergic rhinitis and Eustachian Tube Dysfunction (ETD) leading to ear problems such as Otitis Media (OM). A Pure Tone Audiogram (PTA) and an Impedance Audiogram (I/A or Tympanogram) must be done. Upoload any clinical examination reports regarding the appearance of the nose and ear drums.

2. Chronic sinusitis, if present, along with allergic rhinitis and Eustachian Tube Dysfunction (ETD) causing Otitis Media (OM) should receive a trial of medication initially. She may require a course of antibiotics, pain killers, anti-allergics, decongestants, mucolytics, a steroid nasal spray and steam inhalation. This should be done under medical supervision. These medications take a couple of weeks to reach maximum effect and are more effective when taken together. Only using a nasal steroid spray is not advised.

3. You may share her CT Scan and investigation reports here for more specific treatment options. Balloon Sinuplasty is a newer, less invasive option for treatment of refractory chronic sinusitis. Radio-frequency and Co-ablation assisted surgery are also newer options as compared to FESS (Functional Endoscopic Sinus Surgery). She may require a ventilation tube placement in the ear drum on the right side, depending upon her test results.
