Question: I am 51 yo and had a bad delivery 25 years ago w episiotomy that popped open a week after birth. Dr. Refused to restitch - said it needed to heal from inside out. It never did. Tried estrogen cream but it didn't work. Have suffered painful intercourse for years despite copious amounts of lubricants. Last night, during intercourse, my episiotomy tore open internally - not at the edge - about an inch inside. The tear is at least 1/2 an inch long and bled for hours. Do I need to see my Ob-gyn immediately today, or another one if he is unavailable? I can't use Premarin anymore bc I take Tamoxifen.

I can understand your concern here.
You should visit to your Gynecologist ( or to another one if she is not available) immediately and get yourself examined and suturing the wound if required.
After suturing you can apply steroid cream and hormonal cream( after consulting your doctor) to heal early but right now you should not apply anything on it( it may increase bleeding) .
To control bleeding you can take tranxemic acid.
