Question : I am a 72 year old male with a history of ADD. I have been on a 18 mg Methylphenidate ER daily pill for years. Recently I have been having what I call anxiety attacks where my stomach gets in knots and I feel trapped. I break out in a sweat and cannot sit still to read or do any work. What is going on? Who do I go see? My family practice doctor or my psychologist? Could it be a reaction of the Methylphenidate to some other medicine?

Detailed Answer:
Hello, and thanks for your question.
What you are describing is a classic panic attack. It is a good question as to what is causing it. It would be unusual, but possible, for this to develop on its own at your age. Methylphenidate is known for precipitating anxiety and panic, even though the dose has been stable for a long time. I would first see your primary care doctor for basic bloodstock and an exam, and then if medically cleared I would see your psychologist for further treatment.
