What Does Follicle Size Of 23mm Indicate?

Question: I am currently on day 8 of my cycle and have 4 follicles, my largest at 23 mm. The other are 12 mm or less. I did 3 days of Gonal-F 75 IU day 3-5 and Letrozole 5 mg day 6-10. My doctor told me to stop the Letrozole today and is cancelling my cycle. Isn't a 23 mm follicle good? Two days ago the 23 mm follicle was 14 mm. Wouldn't he want me to do the trigger shot? My estradiol level has also dropped from 241 to 76 in 48 hours. In the past 48 hours my endometrium has gone from 9.6 mm to 6.6 mm. I am so confused!

I read your query and I understand your concerns.

Follicle size of 23 is good. But it had developed very fast and with dropping levels of estrogen and endometrium. Hence it is likely that follicle is not healthy and probably empty.

In such situation your doctor has taken decision to stop the trigger and cancel cycle.

Thank you for your response, Dr. Koregol. In my case, would Lupron be something to consider in my next cycle? I am notorious for having a premature LH surge on Letrozole typically ovulating anywhere from day 8-10. My endometrium cannot keep up with my ovaries. My repsonse to ovarian stimulation has never been an issue. Do you have any other suggestions for that I could discuss with my specialist? Thanks in advance.

Better option would be to start an antagonist injection from day 6 of cycle.
