Usually ear tubes fall off on their own and doesn't need to be removed. Sometimes the fall off goes unnoticed. The timing of fall of depends on the type of the ear condition for which it is introduced and also on the type of ear tube. There are many types like long standing or short duration.
The blood clot seen by gp- it may be a impacted wax also.
