hello doctor,
I am 36 year old married female with alone healthy six and a half year old son, I have been trying to conceive since last one year but have had three miscarriages so far, all in the first trimester, I have been diagnosed with adenoyosis in the fundus and the posterior wall that is 6.5 cms X 4.5 cms X 3 cms. I have a bulky uterus and have passing of clots with severe pain during menses that lasts for five days and my cycle is twenty eight days regular. I conceived this time with my LMP being 25/08/13, but my beta hcg level on 23/09 was 591 iu, on 25/09 it dropped down to 471 iu. I am afraid I will go through another miscarriage. what are the treatment options to carry a full pregnancy in my case , all lab reports are normal , Amh is 1.0

Hi,
With adenomyosis impeding pregnancies to continue, you should stop trying to conceive temporarily and concentrate on treating the adenomyosis first. Local adenomyomas may be removed surgically; ultrasonography as treatment is helpful in some. You should use medication to arrest the progress of the disease. At present, it looks like the fate of the present pregnancy is not good with declining hCG levels. GnRH-a or aromatase inhibitors may sometimes help to improve fertility. Hope your query is answered.
