Hello,
You are having a mild head injury and you will require a cranion-cephalic CT scan in order to rule out any possibility of intracranial hemorrhage which could be fatal if is developing gradual into a large hematoma. Small fracture of the cranium could also have occur. Normally the symptoms related to the injury should have subsided by now or exist in a very very mild proportion.
So I suggest you run to your doctor right away so he can do a quick evaluation and send you for CT.

Hope this helps
