General Surgeon 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Noted the history about your wife and understood the concerns:
One month of anal fissure is a long time to suffer indicative that it has become long-term (chronic) now.

I would advise her following:
Consult a general surgeon for clinical evaluation, actual examination per-rectal digital examination, and proctoscopy if the pain allows. This will give the most proper diagnosis and give the plan of further management.
• Sitz bath in a tub with warm water.
• Continue laxatives and not purgatives, she needs soft stools not loose.
• Antibiotics and other medications as would be advised by her surgeon.

If the medical management fails to give complete relief she will need a surgical procedure like Lord's dilatation or internal sphincterotomy for complete cure and yet to continue with Sitz bath and laxatives. Control of stress and anxiety is a part of management.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant
