How To Deal With Delta Conjugated Hyperbilirubinaemia In A 2 Month Old Baby ?
2 month old male; presents with a week history of progressive yellowing of eyes with a greenish tinge; severe anaemia requiring to transfusions; enlarging head . initial investigation shows delta conjugated hyperbilirubinaemia with derange live enzymes but normal albumin; a low haemoglobin;normal abdominal ultrasound and transfontanelle ultrasoun which shows an enlarged head but normal size ventricles.

If you are asking for a possible diagnosis, from your history it appears to be a disorder of the blood cells, where the blood is being increasingly destroyed. There may be an enlarged spleen as well. The Delta hyperbilirubinemia suggests that there is also an obstruction in the bile excreting mechanism in the liver and/or gall bladder which can be caused by stones produced from excessive pigment production from the breakdown of blood cells. You need to rule out Beta Thalassemia, G6PD deficiency and other forms of abnormal haemoglobin by Haemoglobin electrophoresis.
