Hi,
Welcome to HCM.
Enlarged ovaries won't cause pain.
Your USG report indicates fibroid with endometriosis. Endometriosis/adenomyosis can be the cause of pain. Treatment depends on your age, desire for future fertility. Kindly consult a gynecologist for the same.
In short, if you have completed family, you can go for LNG IUD insertion OR hysterectomy (Removal of uterus). If you are desirous of childbearing, you can go for laparoscopy & removal of fibroid & endometriosis.
Wish you good health.
Regards.
