Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Much more information is needed since if his hands and feet are paralyzed this is not likely to be a simple stroke because of the problem affecting both sides of the body. This is more likely either Metabolic/Autoimmune/Infectious or involving lesion(s) of the spinal cord at a high level in the neck. Doctors will need to evaluate the situation and start appropriate therapy but for me to help I need more information. Does he retain normal sensation in the face, limbs, and trunk. Are his arms/legs involved as well or truly just hands and feet as you've stated. Is he coherent and able to speak or very lethargic and "out of it?" Please provide details.

Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Dariush Saghafi
