Urine Bladder Thickness Increased, Infection Suspected, Urine Culture Shows Sensitivity To Niftran Capsules. Previous Anal Passage Injury. Advise?

Hello Doctor, I had an accident at the age of 15 when I was playing with my friends in the well. Now I am 44. When try to inside the well holding water pipe, my hand got slipped and directly went down freely over the pipe and left side of anal passage got hit with air cock in the motor. Immedeatelly bleeding through my penis like urine. It got continued for 2 days and I got admitted in hospital. Being a small hospital, they gave medicines and injection and I was in Hospital one week and the bleeding got stopped. From that day onwards I get slow discharge of urine and when go for urine. Now I did a health check up and Dr. Told that the urine bladder thickness has got increased and as they suspect infection, I got urine culture done. It shows sensitivity to few things and niftron capsule is given for 2 weeks. Please advise what would be issues and will be alright?

Hello and welcome to HCM.

Thank you for your query.

From the description you have provided, the most possible damage that occurred during the accident you had, is some injury to the urinary tract, that explains the bleeding from your penis as well. With time (one week in your case), the internal injury healed and thus the bleeding ceased.

But, what you need to know is that, every injury in the body heals, with the help of scar tissue. This scar tissue develops after the damaged region contracts (this is a natural defense mechanism, to reduce amount of blood lost). This contraction plus scar tissue might have resulted in narrowing of that part of your urinary tract.

This narrowing of the urinary tract causes only a small amount of urine to pass, thus, the slow stream of urine you have been experiencing ever since. This slow stream prevents the quick evacuation of urine, resulting in some extent of stagnation of urine in your urinary tract.

Any stagnation of urine in the urinary tract, due to various reasons causes bacteria to accumulate in that stagnant urine. The bacteria then affects the surrounding structures, resulting in an infection.

I hope this information is clear. Thus, in my opinion, you would have to consult a urologist who might need to conduct an exploratory cystoscopy and find the region of constriction and place a stent to broaden the passage. This can permanently solve your problem.

I hope I have succeeded in providing the information you were looking for. Please feel free to write back to me for any further clarifications. I would gladly help you.

Best wishes.
