What Causes Dysuria With Pelvis And Rectum Pain?

what does it mean if a teenage boy has pain in the area of the pelvis and rectum when urinating? 15 yrs old, 212 lbs, 5'9", had accident in which fell onto pullstop in shower and the pullstop went into rectum and caused a tear, about 3 yrs ago, otherwise healthy.
