What Causes Neck Stiffness And Right Shoulder Pain?

Hi, Sir, i am 33 year old, i have been diagnosed as i have Ankylosing spondilities, presently i have light neck stiffness, right shoulder pain. Previously before 6 years i had low back pain, after all the exrays, it shows that in 2 places disc space has reduced and low back has narrowed little bit. Presently i cann t bend forward fully, only 45 % i can bend, but i can touch my toes while sitting. When we consulted Immunologist he said my problems are not so major so, need not necessary to start medication like DMD. But when i consulted Rheumatologist, he said it is very serious and should start taking the drugs. i am confused now, whether to go start drugs or not. Please suggest me
