Hello

Your HSG findings suggests uterine fibroid with bilateral chronic salpingitis(long standing infection of fallopian tubes).
There is loculated spillage of contrast from both fallopian tubes.Normally there should be free spillage of contrast from both sides.Locultaed spillage may be due to chronic infection of fallopian tubes.
You need further assessment.USG should be done to assess uterine fibroid. There is possibility of fibroid in left side of uterus.
Treatment of fibroid depend upon many things like age, complications of fibroid, family status etc.
You need treatment from both conditions so that chances of fertility can improve.

Get well soon.
Take Care
