What Does Mild Cognitive Impairment Mean?

my father is a ww2 veteran and has alzheimers. According to his V.A. medical records, it reads that he has G31.84 which is MILD COGNITIVE IMPAIRMENT, so stated. Just wondering what the "so-stated" means. Does this mean that the attending physician was told that the patient had these symptoms or did the physician see the symptoms and this is just how it is written down. Because he is far from having mild cognitive impairment.

It is not easy to diagnosed the cognitive disorders if no obvious symptoms are there even more difficult to differentiate it from other neurological disease if no clear symptoms are there or no medical history is suggestive of any genetic relationship, on other hand if the symptoms are very much clear like dementia, delirium, amentia, then it is the cognitive disorders.
