Fibroids can affect the fertility depending the size, location etc. Large size fibroids within the uterine cavity or near the cervix can affect more by affecting ascent of sperm and implantation. According to your history, the size of the fibroid is not bigger and usually medical treatment either with GnRH analogues or progesterone receptor modulators for few months can help in decreasing the size.
If surgery is decided by your doctor either laparoscopic myomectomy or open myomectomy can be done. And regarding infertility your sister and her husband may need to undergo few more investigations like, For your sister: Antral follicle count at around 3rd day of the cycle, Hysterosalpingogram or sonohysterosalpingogram at around 8th day of the cycle. For her husband: Semen analysis.
With this workup the possible cause of the infertility can be identified and treatment can be taken accordingly.
