Brief Answer:
More tests are needed.

Detailed Answer:
Hello,
The ultrasound of the thyroid suggests of thyroiditis but rather it is Hashimoto (causes under active) or Graves disease (causes over active thyroid) you need to check the thyroid hormones and the antibodies;
-TSH, Ft4.
-Anti-TPO, Anti thyroglobuline, AntiTSH receptor antibodies.
The treatment will be decided after the results.
Tenderness is caused by subacute thyroiditis that is a viral infection of the thyroid gland but in this case the ultrasound would have shown low vascularisation not high.
Please attach the results of the above mentioned tests and i will be happy to help you further.
Kind regards,
Dr.Mirjeta
