Suggest Treatment For Dilated & Obstructed Uterine Tubes

Hello, I am 39 yrs old, am 5 6 138lbs and have had 4 previous pregnancies full term. After having an IUD removed I have tried unsuccessfully to conceive for four months. I am ovulating and my fsh is 9.54. I had an HSG and the results say uterine body appears normal, no filling detected and both tubes appear mildly dialated with poor spillage of the contrast denoting tubal obstruction. Should I have an operation to clear tubes?
